Immunization Requirements for UPEI Health Science Students 

Welcome to UPEI! All first-year Health Science students at UPEI are required to complete the UPEI Immunization Requirements Form. Completion of this form is mandatory and required for continued enrolment in your program.

Obtaining records and completing vaccinations or testing can take time, it is important to begin this process as soon as possible.

Step 1: Gather Your Immunization Records

• Collect all available immunization records from infancy through adulthood before beginning this process.

Step 2: Book an Appointment

• Once records are collected, arrange an appointment with your healthcare provider, Public Health Nursing services, or the UPEI Health and Wellness Centre.
• Completed forms must be submitted to your Program Coordinator by the deadline provided.

• Students living outside PEI who require vaccinations or TB testing may wish to complete these services in their home province or country where costs may be lower or covered.
• The Health and Wellness Centre can assist eligible students with TB testing, bloodwork required for immunity, and most required vaccinations.
