Patient rights:

  • To have the right to consent to or refuse treatment at any time

Patient responsibilities:

  • To attend appointments and be on time
  • To ensure cell phones are off during appointments
  • To treat others with courtesy and respect
  • To come prepared (e.g., bring pill bottles for refills)
  • To be an active participant in their care
  • To follow the treatment plan
  • To participate in protective measures (e.g., to help stop the spread of disease)
