Update - Door Hazard Signage Contact Requirement

As per the April 2023 communication (see here), all laboratory hazard door signs must include two emergency contacts to ensure effective communication during emergencies or situations requiring contacting authorized personnel.

Contact Requirements:

The first contact should be the principal investigator (PI) with Their name, UPEI cell phone number, office room, and office phone number.

The second contact should be a lab user, e.g., area lead, lead hand, or other with their name, UPEI cell phone number, office room, and office phone number. 

*NEW* If the primary and/or secondary above do not provide 24/7 contact information, Security Services can now be listed as a 24/7 contact. In addition to the Security Services contact, the laboratory hazard door signage MUST have a responsible person name and contact number to be contacted during working hours.

To ensure emergency response contact information stays up to date, it is the responsibility of the PI to provide accurate and up-to-date contact information to Security Services. If using Security Services as a 24/7 contact:

  1. The laboratory responsible person MUST provide Security Services with the primary and secondary contact information. Having both primary and secondary contacts allows for reaching someone responsible for the lab while either is away from their phone or on vacation for example. 
  2. The contact information can be a home/work cell or landline phone number that can be reached 24/7. 
  3. The contact information must include the building(s) and room number(s). 
  4. Each laboratory is responsible to:
    1.  email security@upei.ca and cc Nathan Rainnie - nrainnie@upei.ca
    2. include in the Subject line: Laboratory Contact Information - Building/Room(s)
  5. Laboratories must resend contact information whenever there is a change in contact information.  

Maintaining Signage:
Any changes to lab hazard signage must be submitted through a Facilities Management (FM) work order via SchoolDude, including the updated Lab Hazard Signage Form, available on the UPEI Facilities Management page.
