The UPEI Health & Wellness Centre is launching a Provincial ADHD Program for Adults on PEI. The program is funded by the Department of Social Development and Housing and UPEI. The program is seeking a Social Worker as part of the team of professionals who will support individuals who are living with attention deficit hyperactivity disorder (ADHD). The social worker will provide direct service and work closely with other members of the Provincial ADHD Program. The social worker will report directly to the Director of the UPEI Health & Wellness Centre.

RESPONSIBILITIES:

  • Direct clinical service to individuals 16+ who are referred to the Provincial ADHD Program
  • Participate in team meetings, coaching and mentoring sessions and individual/group counselling sessions

The Social Worker will be expected to directly engage in or supervise the following:

  • Providing a wide range of assessment, diagnosis, treatment, and consultation services for individuals who are referred to the Provincial ADHD Program for Adults. The individuals may have multiple co-morbidities, including past criminal behaviors
  • The social worker will assist in developing and monitoring short and long-term plans, and counsel patients on lifestyle issues
  • The social worker will perform Psycho-social assessments – identifying high risk situations to ensure the safety and well-being of individuals and their families/support networks. The assessment includes considering employment, housing finances, supports, etc.
  • The social worker will determine the needs and eligibility for services/resources and explore barriers, identify coping strategies
  • The social worker shall act as an advocate on behalf of the individual and their families/support network. They shall provide support and referrals as appropriate
  • Coordinate and participate in case conferences
  • Receive from and give support to other team members; contribute to the inter-disciplinary effort in improving quality medical/psycho-social care, provide services support and counseling to individuals and their families as they deal with their diagnosis and co-morbidities
  • Follow up with individuals as required to ensure they are meeting their goals; provide education and support to understand their condition, triggers and how to manage impulse control, excessive anger, and behaviors
  • Plans, implements, and evaluates the social work components of the program in consultation with other team members. Plan and implement quality improvement initiatives
  • Maintain and protect confidential records and case reporting consistent with policy, procedures, and ethical standards
  • May liaise with other program/population areas (e.g. Holland College, Provincial Corrections, 2SLGBTQIA etc.). This could include assessing the clinical needs of these communities and augment traditional clinical service with evidence informed clinical outreach
  • Performing other duties as required to facilitate the functioning of the Provincial ADHD Program for Adults, which includes but is not limited to contributing to operational policy development and day to day functioning of the clinic.

QUALIFICATIONS:

  • Applicants must have a Master’s Degree in Social Work, with training and supervised practice with youth (16+) and adult populations
  • Applicants must be eligible for registration with the PEI Social Work Registration Board; where currently registered, must be a member in good standing
  • Prior experience working with adults with ADHD would be considered an asset
  • Ability to liaise effectively with relevant stakeholders (e.g., UPEI/Holland College students, Provincial Correctional Facility, etc.)
  • Demonstrated strengths in clinical work with cultural and other group and personal diversities
  • Demonstrated effective professional skill including time management, interpersonal skills, oral and written skills, effective team building skills, collaborative, and consultative practice skills
  • Working knowledge of Mental Health Act, FOIPP, Consent to Treatment Act, Occupational Health and Safety Act, Youth Criminal Justice Act, Child Protection Act, Health Professions Act, and other legislative frameworks as needed
  • Working knowledge of the Prince Edward Island Social Work Registration Board Code of Ethics
  • The successful applicant must provide a satisfactory criminal record and vulnerable sector check prior to beginning employment
