Reporting to the Provincial Housing Supervisor, the Outreach Worker will support tenants of the Seniors  Housing Program to maintain successful housing placements by achieving and maintaining health and independence through the appropriate quality services. The incumbent will assess tenant needs, determine solutions and in collaboration with the tenant, familt, and service providers support the development of a case plan to implement and monitor solutions.  The incumbent will need to have a strong understanding of the seniors population, those experiencing persistent mental illnesses, addictions, behavioral issues, forensic histories, cognitive disorders, significant mental health issues and/or concurrent disorders.
