Certificate Program Registration Form

Note: Before filling out this registration form, please read the Centre's updated Cancellation Policy/Refund information. After you've read the Cancellation Policy/Refund information, register for a course by completing this form and clicking "Submit" to send your information to our registration coordinator.

Payment Information: Payment of the course fee must be made prior to the start of the course to secure your registration. After your completed registration form is received, you will be sent an email within two days with your payment options. Cash, cheque, Visa, or Mastercard are accepted or your employer can be invoiced if you provide the person's name, title, and address.
