HOW DO I FILE A COMPLIMENT OR COMPLAINT?

You may address it in person at the time of the incident or in writing. If you choose to address it in writing, please complete the section at the end of this page.

CAN I EXPECT IF I FILE A COMPLIMENT OR COMPLAINT?

The box for compliments/complaints will be checked at least monthly. All compliments and complaints are reviewed by the director. Compliments will be shared with the individual and team. Complaints will be addressed appropriately and in a timely manner

WHO ELSE WILL KNOW ABOUT MY COMPLIMENT OR COMPLAINT?

Your privacy and confidentiality will be maintained. You may make your compliment or complaint without leaving your contact information however that will make it difficult to complete all the follow-up. Only staff who are directly involved will know your identity if necessary.
